Nepal's War and Conflict-Sensitive Development

چکیده
Kathryn Maitland, Charles Newton, Kevin Marsh, Mike Levin The study by Planche et al. [1] provides important new information addressing intracellular volume depletion in children with severe childhood malaria, but does not address the question of whether intravascular volume depletion (hypovolemic shock) is present. Using sophisticated methodology to determine total body water and extracellular water, they demonstrate a 6.7% defi cit in total body water and an 11.7% defi cit of intracellular water, providing an important indication of the volumes of fl uid that may be required to optimize hydration. The data, however, do not address the degree of fi lling of the intravascular compartment, nor should they be used to answer the question about the state of tissue and organ perfusion. Indeed, we believe that these new data present no confl ict with our previously reported fi ndings. Using methods to study critical illness physiology that are widely employed within pediatric intensive care units for interpretation of circulatory status, we have demonstrated evidence for hypovolemia in 53 Kenyan children with severe malaria complicated by metabolic acidosis [2]. Our children were younger, had longer capillary refi lling times (>3 s), lower central venous pressures (mean 2.9 cm H2O) and higher creatinines (>80 μmol/l): all features of compensated hypovolemic shock. Furthermore, hypotension (systolic BP < 80 mm Hg) was present in 44% of children with severe acidosis (base defi cit >15). These fi ndings also indicate important baseline differences in two cohorts of children studied. We agree that reconsideration of guidelines for acute fl uid management is warranted, particularly when current recommendations await an adequate evidence base. Nevertheless, confl icting opinions on the question of volume status in children with severe malaria can be satisfactorily resolved only through prospective randomized trials that include both fl uid resuscitation and control groups. While the design and conduct of such trials will involve considerable challenges, optimal fl uid management will never be resolved on the basis of theoretical consideration alone.

Nepal's War on Human Rights: A summit higher than Everest
Nepal has witnessed serious human rights violations including arbitrary arrests, detentions, "disappearances", extra judicial executions, abductions and torture carried out by both the Royal Nepalese Army and the Maoist rebels in the 10 years of the "peoples war". Women and children have borne the brunt of the conflict. Massive displacement has led to adverse social and psychological consequenc...
متن کاملUnderstanding effects of armed conflict on health outcomes: the case of Nepal
OBJECTIVE There is abundance of literature on adverse effects of conflict on the health of the population. In contrast to this, sporadic data in Nepal claim improvements in most of the health indicators during the decade-long armed conflict (1996-2006). However, systematic information to support or reject this claim is scant. This study reviews Nepal's key health indicators before and after the...
متن کاملConflict and Development in Nepal
Despite politically difficult situation and the presence of armed conflict, Nepal's achievements in many socio-economic indicators, especially between 1996, the year when conflict started in the country and 2004, when the intensity of conflict was at a higher level, have been remarkable. The incidence of poverty fell from 42% to 31% mainly because of rapid increase in remittances income, increa...
متن کاملUnderstanding the Rapid Reduction of Undernutrition in Nepal, 2001–2011
South Asia has long been synonymous with unusually high rates of undernutrition. In the past decade, however, Nepal has arguably achieved the fastest recorded decline in child stunting in the world and has done so in the midst of civil war and post-conflict political instability. Given recent interest in reducing undernutrition-particularly the role of nutrition-sensitive policies-this paper ai...
متن کاملEvaluating Community-Driven Reconstruction
RECONSTRUCTION IN NORTHERN LIBERIA presented daunting challenges when the International Rescue Committee launched a community-driven reconstruction (CDR) program there in 2006. The area had been racked by civil war since 1989, when Charles Taylor’s rebel group, the NPFL, launched its attack from Cote d’Ivoire. The conflict was devastating, with 85 percent of the local population displaced at so...
متن کاملGeospatial Science and the Archaeology of the First World War: Context for Conflict
This paper examines the role of geospatial technologies in the development of non-invasive archaeological research methods and relates this development to the expansion of an innovative sub-discipline: ‘conflict archaeology’. Archaeology is predicated on the principle that human behaviour leaves behind physical evidence that is patterned, recoverable and interpretable; while warfare and conflic...
